Specialized Recovery. Safer Transitions. Better Outcomes.
Recovering from a cardiac event, procedure, or hospitalization requires expert care and a focus on safety. Our dedicated Cardiac Rehabilitation Unit (CRU), located within the Woodbury Health Care Center, is a specialized post-acute recovery program designed to bridge the gap between the hospital and a successful return home.
With an average length of stay of 14–21 days, we combine advanced continuous monitoring, cardiac-focused therapy, and coordinated discharge planning to ensure a safe, confident recovery.

Who We Serve
Our program is tailored for individuals recovering from:
- Congestive Heart Failure (CHF)
- Heart Attack (Myocardial Infarction/MI)
- CABG (Bypass Surgery)
- Valve Replacement Procedures
- PCI/Stent Placement
- Cardiac-related weakness, falls, or deconditioning
What Makes Our Cardiac Unit Different?
We go beyond standard rehabilitation.
By utilizing the latest healthcare technology and a multidisciplinary team, we can detect warning signs early, prevent falls, and keep your loved one on track.
Therapy Designed for Cardiac Recovery
Our multidisciplinary team includes cardiac-trained nurses, physical and occupational therapists, dietitians, and MD/NP cardiology oversight.
- Endurance building and strength recovery
- Monitored exercise tolerance

Advanced Cardiac Monitoring & Safety
- Daily Weight Monitoring: In-room scales for daily morning weights to detect early signs of fluid overload or CHF exacerbation.
- EKG Testing: Baseline and scheduled monitoring to quickly identify arrhythmias or ischemic changes.
- Nobi Smart Lamps: AI-powered fall detection and prevention with soft nighttime lighting for safer mobility and faster staff response times.
- Tapestry Health Remote Monitoring: Continuous clinical oversight with real-time vital signs and symptom tracking for early intervention.
Specialized Cardiac Nutrition
Nutrition isn’t an afterthought. It’s part of the treatment.
- Heart-healthy, low-sodium, and fluid-restricted meal planning
- Dietitian-led cardiac education
- Weight- and lab-responsive meal adjustments
Recovery with Confidence: The Cardiac Transitional Kit
The highest-risk period for readmission is immediately after discharge. We ensure every patient leaves with the tools they need for a successful transition home.
Every Transition Kit Includes:
- 3 Days of Heart-Healthy Meals: Low-sodium, cardiac-appropriate meals to prevent early nutrition mistakes.
- Cardiac Care Booklet: Tools for weight logging, symptom tracking, medication reminders, and emergency guidance.
- Coordinated Follow-up: We schedule PCP and cardiology appointments and arrange home health referrals before discharge.
